Bissonette Transition Coordinator
ANNUAL SALARY: $50,000
Position Summary
The Transition Coordinator is responsible for all assigned case-management activities, data collection and reporting for all residents at Bissonette House. The TC provides professional services to participants, staff, employers, and other community and governmental agencies. She/he promotes the programming offered by the agency as well as providing education/vocational counseling and special support services for residents. The TC must be an effective liaison for residents, employers, teachers, criminal justice agencies, mentors, human service providers and residential staff as well as possess a demonstrated ability to network with outside agencies to meet participants’ needs and promote opportunities provided by the community and Peaceprints.
Essential Functions
The functions listed below are representative of the knowledge, skill and/or ability required. Other duties may be assigned.
- Participates in professional development and training with a demonstrated understanding of agency needs and expectations both in general and as they relate to specific contractual conditions relating to grant and other funding.
- Uses effective communication tools for case management functions, data collection and reporting as well as case management systems and documentation.
- Networks, establishes and maintains relationships with community service agencies, as well as stakeholders in education, training and employment and housing.
- Conduct and develop Individual Service Plans (ISP) for residents and oversee the enrollment process.
- Maintains a referral network for support services and identifies new contacts and resources in response to residents’ needs.
- Plan weekly case conferencing with Program Manager.
- Serves as case manager for an assigned participant group providing:
- Testing as appropriate to determine interests, skills and appropriate placement
- Works closely with residents to identify therapeutic needs, educational/vocational goals, job readiness, employments barriers and available resources
- Makes referrals and maintains documentation of referrals, benchmark attainments, level of participation and termination/completion
- Monitors residents’ progress and modifies ISPs as necessary
- Provides timely follow-up with residents, graduates, employers and/or potential employers to obtain placement outcomes, and exhibits persistence in attempts to contact for current employment information (via telephone, e-mail or other communication methods)
- Coordinates with program instructors, volunteers and service providers to plan externship site visits as necessary
- Has working knowledge of training, employment, education and labor market materials
- Tracks progress and attendance, and initiates communication with residents and the appropriate service providers to identify and address potential challenges
- Maintains accurate and confidential resident records according to internal classifications and consistent with all applicable state and federal statutes
- Ensures that all enrolled participants provide the appropriate documentation
- Records and maintains documentation of all participant counseling sessions
- Assists in teaching daily living skills and provides ongoing assessment and feedback to residents
- Screens residents for community service/restorative justice programs and assists in documentation and reporting of agency participation
- Actively demonstrates a respect for all Peaceprints residents and embraces the values of the agency
- Participates in the development, delivery, and promotion of services offered by Peaceprints
- Follows established procedures and guidelines
Education and Experience
Minimum associate degree in education, Social Work or a related field. Master's Degree preferred. Minimum of 2 years' experience in education, social, or human services. Computer proficiency required.

Position requires some travel within the City of Buffalo.

Must be able to pass criminal background and approval through the Department of Corrections and Community Supervision.
Physical Requirements
- Limited local travel is required. Light physical effort in an office environment
